Identify A Health Maintenance Organization (HMO) in the CMS Medicare Advantage Plan Directory
You can find out the name, address and contact information for a HMO in just a few steps.
- Go to the CMS Medical Advantage (MA) Plan Directory page on the CMS Web site
- Click on the current MA Plan Directory link. The link will prompt a window to open a zip file. Open the CSV spreadsheet document in the zip file
- Hold down Ctrl + F on the keyboard to open the Find/Replace popup window
- Type in the HMO Code you wish to identify and click on Find Next to search for the entered code
