“Certificate of Registration”, renewed annually, issued by the FL Dept of Revenue (850) 488-6800. (For diabetic footwear only)This permit is not required if supplier only sells items that require a doctor’s prescription. However, if they are providing off the shelf diabetic footwear the supplier is required to obtain a tax permit.

|Orthotist/Prosthetist Licenses? Yes|
If the supplier is selling diabetic footwear, one of the following licenses is necessary: Orthotic Fitter, Orthotist, Pedorthist or Prosthetist-Orthotist; these licenses are renewed February 28 every odd year and are issued by the FL Board of Orthotists & Prosthetists, State of Florida Dept of Health, Division of Medical Quality Assurance (850) 245-4355. A Podiatrist license is also acceptable. (See Medical License requirements.) Pedorthotist License is acceptable as long as the orthosis is below the ankle (and foot). Anything above the ankle is outside the scope of a Pedorthotist and would require an orthotist license.
