JSM/TDL-08250 recently announced a revised version of the CMS 855S application for DMEPOS suppliers. Starting July 1, 2008, suppliers must use the revised version of the 855S (02/08). The revised CMS 855S added supplier standards 22-25 regarding accreditation to the list of standards for which suppliers must comply in order to receive and maintain billing privileges. The 06/06 version of the CMS 855S will be accepted through June 30, 2008.
The Centers for Medicare & Medicaid Services (CMS) has placed the revised enrollment application on the
CMS Provider Enrollment Web site. The application version appears bottom left in the footer of the application form.
