In addition to the
Helpful Hints on Completing the CMS 855S, the information listed below will assist suppliers with completing the reenrollment process.
- The reenrollment may be signed by either the authorized or delegated official listed on the supplier file
- Suppliers must ensure a copy of the notification received from the NPI enumerator is included with the submitted package. As stated in the letter that accompanies the reenrollment package, suppliers are required to submit this documentation. Please note, the NPI notification must also have the business name and physical address of the location reenrolling. If not included, the NSC will have to request this information, which will delay processing.

Applying for the NPI is a process separate from Medicare enrollment. To obtain an NPI, you may apply online at
https://NPPES.cms.hhs.gov. For more information about NPI enumeration, visit

- The letter that accompanies the reenrollment package also requests that suppliers complete and submit the CMS 588 form if they are not already receiving payments electronically. Please note, the NSC neither processes these forms nor enrolls suppliers into the electronic funds transfer program. The NSC simply forwards the CMS 588 forms to the appropriate contractor for processing. Suppliers should contact their respective DME MAC if they have any questions regarding this process.
- List a contact person in section 13 of the CMS 855S. This will assist the analyst when additional information is required to complete the reenrollment process. This contact person should be someone who is knowledgeable about the information listed on the CMS 855S and who can be easily contacted. Often, there is no contact person listed and the analyst is required to contact the authorized or delegated official on file, who may not be available. Also, please be aware that the NSC analyst is only able to request information from the contact person during the processing of the reenrollment. Once processing is complete, the contact person may not access to any information on the supplier file.
- Provide contact information for the underwriter to verify insurance coverage. This information is now required in section 2D of the CMS 855S (06/06) application form. Providing this information will facilitate the verification process.
- Although most of the CMS 855S form is already completed with the information on file, there are some areas that are left blank for security reasons. For example, the social security number and date of birth is not preprinted for individuals listed in Section 6. This is a precaution taken by the NSC to prevent the possibility of the information being used fraudulently. However, section 1124A of the Social Security Act require suppliers to provide this information. Please note sections 2D, 6B and the tax identification number for the billing agency listed in section 8 are also not preprinted. Please complete this information to avoid delays in processing.
