Long-term Cardiac Monitoring

The Holter monitoring code family includes CPT code ranges 93230-93233 and 93235-93237. These codes are used to report a single 24-hour period or, on rare occasions, a 48-hour period. Palmetto GBA does not consider a repeat Holter test within a 180-day period to be medically necessary, except to ascertain a response to a change in treatment based on a Holter monitor recording.

Dates of Service (DOS) and CPT Codes:

  • Submit claims for long-term cardiac monitoring services for time periods greater than 24-48 hours with the appropriate cardiac event CPT code from the appropriate code ranges (see CPT codes 93268 - 93272 and 93012 - 93014)
  • The descriptions and Medicare reimbursement reflect 30-day intervals for these codes. Please review the CPT descriptions to ensure the code you select describes the service you provided and device used. If you identify any claims that were submitted inaccurately, you may request a 'reopening' and Palmetto GBA will correct the identified claims. 
  • Report global services with CPT codes 93224 or 93230, using the date of physician review as the DOS
  • Report physician review and interpretation only with CPT codes 93227 or 93233, using the date of physician review as the DOS.
  • Report services for the recording only with CPT codes 93225 or 93231. Report claims for analysis with report only with CPT codes 93226 or 93232. In both instances, submit the date the service was performed as the DOS.
