Invoices must be submitted with claims for HCPCS code V2785 (processing, preserving and transporting of corneal tissue) in order for Palmetto GBA to determine the appropriate payment. Payment is based on the lesser of the billed amount and invoice amount. Charges for shipping and handling are not covered.

Electronic claims:

• Enter the word 'Fax' in the documentation record (Loop 2300, or 2400, NTE, 02).
• Include a fax cover sheet for each claim documentation.
• Fax the actual invoice with the claim. Include a fax cover sheet for each claim documentation.
• Fax documentation on the same day the electronic claim is submitted or one to two days prior to the submission of the electronic claim. Include a fax cover sheet with each document.
