Permanent Nail Removal (Partial or Total)
CPT code 11750 (Removal of nail bed) should be used when a partial or total permanent nail removal is performed by one or a combination of the following methods:
- Phenol Alcohol
- Sodium hydroxide
- Negative galvanic current
- Electrodesiccation
- Thermocautery (Bovie)
- Onychotripsy
- Laser technique
- Sharp dissection (e.g., Suppan nail technique, Frost, Winograd, Sibley, Heifetz or similar procedure)
- CPT code 11765 (Excision of nail fold, toe) should be used when excision of skin and suture repair of the nail fold is performed
- CPT codes 11760 (Repair of nail bed), 11762 (Reconstruction of nail bed) and 11765 (Excision of nail fold, toe) should not be submitted for payment in addition to CPT code 11750
- CPT Code 11765 does not include a partial or total matrixectomy and should be used only when the skin is excised along the nail margin or nail fold
- CPT codes 11750 and 11765 apply to one or both sides of the nail or nail fold, or the entire nail or nail fold
- Sides should not be submitted for payment separately
- The number of services submitted should be one
- Exact toe locations should be indicated by using the appropriate modifiers
