Provider Enrollment Applications: Online Status

You can access the status of your submitted provider enrollment application (CMS-855 form) via the Palmetto GBA Web site. The Provider Enrollment Application Status Lookup tool will tell you if your application is open (received and pending) or closed (completed or returned for more information).  
 
To access the status of an application you have submitted, enter your existing Provider Transaction Access Number (PTAN) or the Document Control Number (DCN) from any enrollment-related correspondence mailed to you by Palmetto GBA.

Tips for using the Provider Enrollment Application Status Lookup:
  • Please allow 15 days after submitting your application to check the status using this tool
  • If you do not receive status information using the number you entered, call Palmetto GBA and we will research the status of your enrollment application
    • Ohio and West Virginia providers: call the Provider Enrollment Support Line at (866) 308-5439
    • South Carolina providers: call the Provider Contact Center at (888) 828-2092
