Cross reference (Xref) document control number (DCN) has been changed to an invalid DCN or for a cancel (xx8 type of bill) the Xref DCN does not equal the cancel DCN. Correct the invalid DCN and resubmit the claim. If the claim was returned with reason code 30955, let claim fall from the ‘T’ purge and resubmit a new xx8 TOB with the correct information.

This reason code is received when an incorrect DCN was used on an adjustment. Verify the Health Insurance Claim (HIC) number, Xref DCN, dates of service, and provider number on your claim. Once verified, resubmit a new claim.
