Guidelines/Instructions:
- This modifier may only be submitted with Evaluation and Management (E/M) procedures rendered between 10/01/2008 through 12/31/2008
- Submit this modifier when the E/M service is prolonged or the service is otherwise greater than that usually required for the highest level of E/M service within a given category
- This modifier was deleted for services rendered on or after 01/01/2009
