Guidelines/Instructions:
This modifier is used for hospital outpatient reporting purposes and is related to utilization of facility Reference.
- This modifier may not be used for physician reporting of multiple Evaluation and Management (E/M) services performed by the same physician on the same date
- This modifier is valid for the following CPT code ranges: 99201 through 99239, 99241 through 99255, 99281 through 99299
- Submit this modifier to report circumstances involving E/M services provided by a physician in more than one (multiple) hospital outpatient settings
