Your Attention Please - Mailing Process Changes for Part A Medical Review Correspondence
Palmetto GBA Part A medical review is moving to a new mailing process. All medical review correspondence will be sent to the
Correspondence Address. This is the address reported in Section 2C of the Centers for Medicare & Medicaid Services (CMS) Form 855A (page 9). If there is no
Correspondence Address on file, medical review correspondence will be mailed to the
Practice Location Address. This is in the address reported in Section 4A of the CMS Form 855A (page 17).
The Part A Medical Review department will no longer be able to forward correspondence to a mailing address other than the
Correspondence Address or
Practice Location Address submitted to Provider Enrollment by way of the CMS Form 855A and currently available in the Fiscal Intermediary Shared System (FISS).
Some medical review correspondence may be time sensitive and require a response. It is imperative that providers ensure appropriate staff, at the
Correspondence Address or
Practice Location Address location are educated regarding the processing of incoming mail received from Part A Medical Review, in order to identify the appropriate destination or person to which it should be forwarded.
Examples of this type of mail may include, but is not limited to, the following:
- Additional Development Requests (ADRs)
- Teaching and Instruction to Provider (TIP) Letters
- Provider Edit Effectiveness Letters

Correspondence Address or
Practice Location Address, refer to the 'Tips to Facilitate the Medicare Enrollment Process for Institutional Providers (CMS-855A)' section of the CMS-855A form on the Palmetto GBA Web Site.
