A written request must be submitted to the assigned Palmetto GBA reimbursement consultant requesting the change.
Change in Intermediary
A written request must be sent to the appropriate CMS Regional Office requesting the change.
Change in Area Code
A letter may be submitted to the provider enrollment area notifying them of the change in area code.
Change in Zip Code
A letter may be submitted to the provider enrollment area notifying them of the change in zip code.
