Identify a Provider

Provider ID Information is available on the Centers for Medicare and Medicaid Services (CMS) Web site for Hospitals, Skilled Nursing Facilities (SNFs), Renal Facilities, Hospices, and Home Health Agencies (HHAs). This is especially helpful if you have an overlapping claim situation. You can access other providers' contact information on the CMS Web site.

1. From the CMS home page, click on Research, Statistics, Data, and Systems.
2. In the Files for Order section, click on Cost Reports.
3. On the Overview page in the small box to the left of the page, click the appropriate line of business for the provider you are searching for.
4. Scroll down to Frequent Reports. Look for either Name and Addresses or Provider ID and open the spreadsheet to view the list of providers.

Note: This information is updated frequently so we suggest you use the file as a Read-Only file and access the Web site each time you are identifying a provider.

Hospital IDs

1. Scroll to Downloads section
2. Under Frequent Reports, click on Hospital Names and Addresses.
3. Open file to view list
4. Press Control/F to use the Find function
5. Type the provider number and click Find Next

SNF IDs

1. Scroll to Downloads section
2. Under Frequent Reports, click on SNF Provider ID Information
3. Open file to view list
4. Press Control/F to use the Find function
5. Type the provider number and click Find Next

Renal Facility IDs

1. Scroll to Downloads section
2. Under Frequent Reports, click on Renal Provider ID Information
3. Open file to view list
4. Press Control/F to use the Find function
5. Type the provider number and click Find Next

Hospice IDs

1. Scroll to Downloads section
2. Under Frequent Reports, click on Hospice Provider ID Information
3. Open file to view list
4. Press Control/F to use the Find function
5. Type the provider number and click Find Next

Home Health IDs

1. Scroll to Downloads section
2. Under Frequent Reports, click on HHA Provider ID Information
3. Open file to view list
4. Press Control/F to use the Find function
5. Type the provider number and click Find Next
