Computerized Corneal Topography: Referring/Ordering Physician Required
When submitting CPT 92025 (computerized corneal topography), the name and PTAN of the referring/ordering physician must be submitted in the designated claim fields.
If submitted without the above information, CPT 92025 will be rejected and must be resubmitted as a new claim with the missing information.
