NPI Development Letters: Rendering and Billing PTANs Needed

Palmetto GBA must request your legacy identifier information when we cannot find a one-to-one match for your National Provider Identifier (NPI) and your Provider Transaction Access Number (PTAN- also referred to as the 'Medicare legacy number').

These letters require specific information to be completed and returned to Palmetto GBA in order for your claims to continue processing. Below is an example of the critical portions of this letter that we want to bring to your attention:

Note: You must complete this information for each claim ICN listed on your development letter and return it to the address listed at the bottom of your letter. Once the letters are completed and returned with all the requested information, we will be able to continue processing your services.

When a one-to-one match for your NPI/PTAN combination cannot be determined, the development letters are generated to the first active Medicare PTAN for your Tax ID. If your records show that the patient(s) listed is not yours, please confirm that the patient is not being seen in a different location for your practice.

If you would prefer not to receive these letters in the future, you may choose to collapse your PTANs into a single number. This will reduce the number of PTANs tied to your single NPI, so we can have a one-to-one NPI/ PTAN match. If you have multiple PTANs that are tied/linked to different tax identification numbers, you will not be able to collapse down to one PTAN for all of your locations.
