CPT Code: 66821
Bilateral:
- Use CPT modifier 50 if performed bilaterally
- Exception: Do not use CPT modifier 50 in conjunction with CPT modifier 78. Use HCPCS modifier(s) RT and/or LT
Unilateral:
- Use HCPCS modifier RT or LT if performed unilaterally
Performed following Cataract Surgery:
- Use CPT modifier 78 if performed within three months of cataract surgery
- Do not use CPT modifier 50 with CPT modifier 78, but instead use HCPCS modifier RT and/or LT in addition to CPT modifier 78
Removal of opacified vitreous or vitreous membrane:
- Do not use CPT code 66821
- Use CPT code(s) 67005 or 67036-67040
