The National Uniform Billing Committee (NUBC) maintains codes required when using the UB-04 form and includes revenue, condition, occurrence and value codes.
More information on basic coding rules can be found below:
•Use only codes that are valid for the date of service
•Link CPT codes to revenue codes when required
•Follow Outpatient Code Editor (OCE) guidelines when required
•Follow all guidelines for diagnosis coding. Special attention should be given to the following requirements:
•Diagnosis codes must be coded to highest specificity required for each code
•Refer to diagnosis code guidelines to determine if a diagnosis code can be billed in the primary, secondary or either position
