Issued through CA Dept. of Health, 916-322-2632 All DME suppliers must have a HMDR , “Home Medical Device Retailer”
EXCEPTIONS: If O and P Only, no HMDR is required. Manufactures, wholesale distributors, if not selling directly to patient, licensed pharmacies, licensed nursing home, physician, or hospital; or providing automated external defibrillators (AEDs) only.
EXCEPTION: A physical therapist can prescribe and provide TENS UNITS to patients. If the physical therapist has a DME business separate from the practice, he would need both the HMDR license and Exemptee license.
Additional: MUST HAVE a EXEMPTEE HMDR , or a Pharmacist license, if providing the following items: Legend devices (RX only label), oxygen, TENS units, CPAP, Nutritional feeding kits (w/syringes), IV pumps. (Any item that has a caution (legend) that is only for sale by or by order of a physician. It is not an item that can be purchased over the counter but must be prescribed.) The EXEMPTEE or Pharmacist must be an employee and under full control of these items during dispensing of prescription devices.
