Description
Revenue code 44x is present on the claim; however, occurrence codes 45 and 11 are not present, or occurrence codes 45 and 11 are present and revenue code 44x is not present.
Applicable for type of bills: 13X, 23X, 34X, 74X or 75X.
Resolution
Revenue code 44x is used to indicate charges for services provided in speech language pathology and should be used in conjunction with occurrence code 11 to indicate the date the patient first became aware of the illness/symptom and occurrence code 45 for the date speech language therapy began. Please make the appropriate corrections and resubmit your claim.
