Written Inquiries sent to Palmetto GBA

Every year, Palmetto GBA receives hundreds of inquiries that we cannot answer because the request is not clear. When you send a written inquiry to Medicare to obtain confidential information, the Centers for Medicare & Medicaid Services (CMS) regulations require you to provide the authentication information below:

  • National Provider Identifier (NPI)
  • Provider Transaction Access Number (PTAN)
  • Tax Identification Number (TIN): last five digits

To ensure efficient processing of your request, please complete the Written Inquiry Request Form; this is an interactive Adobe Acrobat file, which allows you to complete and print it from your computer. This form can be mailed or faxed.

Please note!
It is very important that your staff be aware of this provider authentication requirement. Written inquiries that include requests for confidential information that are submitted without the required authentication information will not be completed.
