Prior authorization requests receiving anything other than affirmed or non-affirmed decisions result in a decision letter being mailed to the correspondence address on file. If a prior authorization request is determined to be incomplete, a letter will be mailed to the address on the request form. Decision letters for affirmed and non-affirmed decisions are either faxed or mailed to the address as indicated on the prior authorization request form that was submitted. This means the address or fax on the prior authorization request form may be a different place than the correspondence address on file.
