Outpatient Department (OPD) Prior Authorization (PA) Required Documentation

When submitting an OPD PA Request, the following information must to be submitted with each request:

1. Documentation that the specific risks and benefits of the procedure were discussed with the beneficiary
    
2. Documentation that alternatives and non-surgical options were discussed with the beneficiary
    
3. Clear documentation that the beneficiary desires surgical correction
