During the Part A East (PAE) telephone discussions with Qualified Independent Contractor (QIC) C2C Innovative Solutions, the face-to-face encounter emerged as the top reason home health claims were found unfavorable: no clinical encounter notes and/or face-to-face form to support homebound status.
