Providers billing for only the interpretation of a diagnostic test (diagnostic X-rays, laboratory tests, and other diagnostic tests furnished), must, when requested, submit documentation to support the order and medical necessity of the test. This may require that the performing provider obtain a copy of the order or authenticated (signed) medical records from the ordering provider or the provider that referred the test for interpretation. Failure to provide this information when requested will result in an error being called and collection of any overpayment.
