Reason Code 39929

Description
All line items on the claim are rejected or rejected/denied.

Resolution
Line item rejection/denial information can be obtained from the remittance advice or via the Direct Data Entry (DDE) system. Review the reason for rejection/denial and verify the information submitted on the claim. To find the line level reason codes:

  • From claim page 2, press PF11 twice
  • A line level reason code will be shown in the Denial Reason Field or Line Item Reason Codes Field
  • Press PF1 to access the reason code file
  • Key line item reason code over claim level reason code, and press Enter to view narrative
    • After reviewing press PF3 to return to claim
  • Press PF10 twice to go back to left view of claim page 2 or press PF8 to go to page 3

If an error on the claim caused the line item rejection, you may submit an adjustment claim to correct the error. Line item denials are usually medical necessity denials. If you disagree, you may request a redetermination (first level of appeal).
