A Local Coverage Determination (LCD) is denoted as superseded on the CMS website. Please provide me with the advisory, notice or policy that gives the verbiage which supersedes this LCD. It is rumored that the supporting diagnosis codes have been revised, but there is no record I have found to verify this.

Answer: Each time the LCD is revised the previous version, which is located at the end of the LCD, is marked “Superseded.” In the absence of a national coverage policy, an item or service may be covered at the discretion of the Medicare contractors based on a local coverage determination (LCD). All CPT and ICD-10 CM codes shall be removed from LCDs and placed in billing and coding articles or Policy Articles that are to be published to the MCD and related to the LCD.
