Need to submit a license but you don't want to submit a CMS-855S enrollment application? To better serve you, the NSC has created an online tool for you to submit licensure documentation to the NSC! The NSC has created an enhancement for the NSC website to upload license(s) electronically without submitting a CMS-855S enrollment application. You may use our new online tool to submit current licensure documentation to the NSC in PDF format. By simply entering your identifying information in the designated fields, you may attach your licensure documentation and simply click "submit". A change of information is not required!
Simply, go to the NSC website (www.palmettogba.com/nsc), go to "Forms/Tools", then click NSC Licensure Documentation Web Form.
