Coumadin (any setting) and Heparin (hospital setting only) are considered high risk management options because they are drugs that require intensive monitoring for toxicity.

To be considered high risk management options, the patient’s drug levels must be monitored at appropriate intervals and documented in the patient’s medical record. This supports the provider’s involvement in the patient’s care.
