HCPCS modifier GQ is used to report services delivered via asynchronous telecommunications system.
Guidelines and Instructions
This modifier may be submitted with telehealth services. Generally, interactive audio and video communications must be used to permit real-time communication between the distant site physician/practitioner and the Medicare beneficiary. The patient must be present and participating in the telehealth visit. The only exception to the real-time communication requirement is for federal demonstration projects conducted in Alaska and Hawaii.
This modifier may only be submitted with codes listed on the CMS Telehealth website.
The following types of medical practitioners are permitted to perform telehealth services. This modifier may only be submitted by these practitioners:
Clinical nurse specialist
Clinical psychologist (with some restrictions: refer to CMS Pub. 100-04 as described in the Reference section below)
Clinical social worker (with some restrictions: refer to CMS Pub. 100-04 as described in the Reference section below)
Registered dietician or nutrition professional
