All Credit Balance Reports for the quarters ending March 31 and June 30, 2020, are due on July 30. On July 15, 2020, Palmetto GBA issued Credit Balance Report reminder letters to providers who we did not receive a report from as of the date of the letter.

Providers are encouraged to submit their Credit Balance Report via Palmetto GBA’s eServices. Providers can electronically submit the Credit Balance Report and PDF attachments online. Once the form is successfully submitted, you will receive an inbox message advising you that the form was received by Palmetto GBA. You may access the form via the Financial Forms subtab located under the Financial Tools tab in eServices.

Providers may also submit by fax to (803) 419–3277. It should be noted that this method will not provide confirmation that the submission was received.

You can see the status of your Credit Balance Report(s) through our Credit Balance Report Look Up Tool. The tool will provide you with our internal Document Control Number (DCN), the status of the report, the date it was received at Palmetto GBA, and any applicable comments.
