Answer No. If the minimal documentation requirements are not met for a new patient (CPT codes 99200–99205), the provider must submit the unlisted Evaluation and Management (E/M) CPT code 99499 with supporting documentation. You may not use established patient codes (CPT codes 99211–99215) for new patients when the minimum documentation requirements are not met either.
