On Friday March 30, 2018, Palmetto GBA sent Provider Enrollment (PE) acknowledgment emails to all Jurisdiction J (JJ) Part B providers with a pending application on file. Providers that did not enter a valid email address on the CMS Form 855 will receive an acknowledgment letter via regular mail. This correspondence serves as notification and confirmation that Palmetto GBA is in receipt of your enrollment application received from Cahaba as part of the JJ transition. For your convenience, you may use our self-service tools to check application status both by phone and online. Both methods are easy to use and available 24/7.

To check your application status by phone:

  1. Call Provider Contact Center Interactive Voice Response (IVR) at 877-567-7271
  2. Press 2 for Provider Enrollment
  3. Press 3 for Part B
  4. Press 1 for PE Application Status Lookup (You will be prompted to enter the DCN)

To check your application status online:

  1. Access the Palmetto GBA JJ Part B website
  2. Click on the Forms/Tools tab at the top of the page
  3. Select the Enrollment Application Status Lookup Tool.
  4. Enter the Document Control Number(s) (DCN) listed on the email or letter. Palmetto GBA will then show you any applications that are pending, and also let you know if your application has been completed or still pending.
