Claim Documentation Submission Made Easy with Additional Documentation eForm

Palmetto GBA is pleased to provide you with a convenient, secure method of submitting your claim documentation. Using our secure online eServices portal, providers can upload a variety of documents such as operative reports, invoices, etc. based on pre-selected criteria. The form includes drop-down menus of the valid CPT codes and modifiers allowed for documentation submission, giving providers the ability to send additional documentation to support specific claims submission scenarios and prevent submission of unnecessary documentation. This ensures you are sending the exact information needed for correct processing of your claim. eServices saves users time and money, and more importantly – ensures the timely delivery of your documentation to Palmetto GBA. Uploading documents is as easy as attaching a document to an email. The Additional Documentation Form is located under the Claims tab.
