Reason Code 32403

FISS Narrative:
According to the revenue code table, a HCPCS is required for the line item being edited; however, the statement coverage dates on the claim fall outside of the effective/termination dates for the HCPCS on the HCPCS table file, so the HCPCS was not valid during the statement coverage period.

Explanation and Suggestion:

• Reason code 32403 is very similar to reason code 32402
• Please refer to the most current version of the CPT-4 HCPC coding book for the HCPCS required
• Online providers may access the HCPCS file for a list of revenue codes allowed with the HCPCS billed
