Guidelines and Instructions
Submit this modifier to indicate the origin or destination of an ambulance transport when the location is a SNF. The patient's residence is the place where he or she makes his or her home and dwells permanently, or for an extended period of time. In contrast, a SNF is one that is listed in the Directory of Medical Facilities as a participating SNF or as an institution which meets section 1861(j)(1) of the Social Security Act.
