E/M Weekly Tip: Unable to Obtain (Examination)

The acuity caveat (unable to obtain) does not apply to the examination component; details of what efforts were made to assess the patient, along with an explanation of why the area of focus could not be assessed should be explained to gain any credit for “unable to assess.”
