Description
Status code 42 for a hospice discharge, is only valid when the bill type is 81x or 82x. The fourth digit of the reason code reflects the sequence in which the error occurred. The sequence is 1 through 0 (first through tenth occurrence) and A-T (11 through 30 occurrence).
Resolution
Occurrence code 42 (date of discharge) is only valid for Medicare hospice claims when used with type of bill 0811, 0814, 0821 or 0824. Please correct and resubmit the claim.
