A - If the supplier is providing medical gas to other licensed entities, such as hospital clinics, or other distributors, a “Wholesale Distributor License” is required. If the supplier is providing medical gas upon prescription to patients, then a Supplier of Medical Equipment, Legend Drug and Medical Gas License is required.

B - “Supplier of Medical Equipment, Legend Devices, or Medical Gas” permit, with expiration date, issued by the AR Board of Pharmacy, at 501-682-0190. Not required by health care professionals licensed in AR who practice within the scope of their profession to prescribe and/or dispense those devices (i.e. Physicians, Physical or Occupational Therapists, Respiratory Therapists), or if only selling orthotic/prosthetic items, canes, crutches, walkers, commodes, or manual wheelchairs.

C - If doing custom items, must have an Orthotist/Pedorthist/Prosthetist License issued by AR Dept. of Health. The license can be verified at: http://www.healthy.arkansas.gov/opp/verification/index.php. A license is required for out-of-state suppliers.

D - A “Supplier of Medical Equipment, Legend Devices, Medical Gas Permit” is required if the supplier provides electronic or computer-driven wheelchair seating systems/cushions.

E - To dispense eyeglasses in Arkansas the supplier must hold a dispensing optician license/registration and an office permit. These licenses shall not apply to Arkansas licensed Ophthalmologists or Optometrists. The fitting, dispensing, selling, or supplying of contact lenses must be done by a licensed Optometrist or licensed Physician skilled in disease of the eye.

Z - As of 2008, Cochlear Implants are no longer considered DME, and they are now paid through Part B
