A - If a supplier is shipping to the end user, a Utah license is required. The pharmacy classifications are as follows: -Pharmacy Class A: Retail Operations in Utah -Pharmacy Class B: Institutional Pharmacy (Closed Door, Hospice Facility, Sterile Product Preparation) -Pharmacy Class C: Manufacturing, Producing, Wholesaling -Pharmacy Class D: Pharmacy located outside the state of Utah -Pharmacy Class E: Medical Gases, DME

B - A Utah Controlled Substance License can be attached to Classes A through D which will enable a pharmacy to provide Controlled Substances.

C - A Medical Supply Company providing Durable Medical Equipment or prescription devices to Utah residents, whether located in Utah or located in another state, must be licensed as a Class E Pharmacy in Utah. Utah Pharmacy Practice Act Rules specifically state that Class E pharmacies do not require a Pharmacist-In-Charge.
